CMA ?

In the ICS Cleaning Specialist December 2006 issue page 24-26, Beating the Wintertime Blues Part 2 talked about if the DM say's yes to a proposal you bring out your CMA. What exactly is a CMA, I know it is a Carpet Maintenance Analysis sheet. Does anyone have an example of one on hand? Please explain here and may send me some samples to me e-mail.
