Formal Safety Program Form

Hello to all,

I'm in the process of re-launching my Business and need to come up with a (simple) formal Safety program Form that my employees can sign, for the purpose of WC insurance.

Can anyone point me in the right direction?

PS I had to register again My old profile(tadsvces) would not let me log-in.

This can be a one page document. Make it general.

1) safety first and always, safety is NEVER to be sacrificed for production.

2) proper Personal Protective Equipment (PPE) is to be worn as needed.

3) where you keep the MSDS book on each truck

4) where you keep the first aid kit on each truck

5) who to call and telephone number

6) regular training (these need not be long, a short start of the day blurb covering proper lifting, mixing chems, defensive driving, etc 5 - 10 min tops)

Cover what is expected (work safely!), what happens if the rules are broken, what PPE the company provides, what PPE the employee is expected to provide, what to do if unsure how to proceed, what to do if asked to operate unfamiliar equipment, etc
