Has anyone ran into this problem in the pass when doing any type of tile cleaning or concrete cleaning.

How do you prevent it and remove it?

I don't think Dave gets on the boards much anymore. I always use a T&G Acid cleaner and it's always taken care of efforescence. However there is a chance it will return over time. Hopefully the grout has cured and one application will take care of it.
BTW I'm assuming your talking about T&G not natural stone, don't use the Acid on most any stone products.

You might check out Aldon Chemical Company in Tennessee they have an efro product that works very well.
